When we take on a case we speak daily with patients, family members and caregivers to deliver the best quality care. We supervise medication management and work with social workers to arrange discharges and follow-up appointments.

FIND THE RIGHT PHYSICIAN / SPECIALIST

With over 25 years of  experience, we have direct access to the best physicians and surgeons around the country.  If patients need close monitoring and frequent testing, we will keep medical staff informed of any daily concerns, possible medication interactions and other important issues. When hospitalized and at home recovering we will maintain close liaison with the medical team to assure continuity and good follow-up care with home care providers.

CASE MANAGEMENT

We guide patients and their families through the complicated health care system. This includes help in screening, diagnosis, treatment and follow-up care. We coordinate medical tests with your providers, and communicate daily with doctors and nurses to achieve the best possible outcome. Monitoring your medication for potential interactions and side effects is another important role we can play. Before discharge, we can obtain prior authorization for homecare services. 

BILLING AND INSURANCE ISSUES / ADVANCE CARE PLANNING

In addition to the guidance and support of a seasoned patient advocate, health emergencies can be handled best if patients have a health proxy and other related materials in place. We can refer you to elder law attorney Steve Fleischer, whose expertise includes Medicare and Medicaid enrollment, health proxies, wills and estates, as well as advance care planning. With advance care planning, there will be fewer surprises and there will be instructions already in place for handling end-of-life issues.
